Mail Form Builder

Just input the sender`s name and email address, and this form does the rest for you:
- Creates a form to send email from your website.
- You can choose which address fields are hidden, and which address fields your user fills in.
- You can fill in default values for all fields.
- More to come, like perhaps a generator for multiple fields you would like emailed to you (currently, fields supported are only to/from information)
